Employment Type: Part time Shift: Day Shift Description: Assesses learning needs, readiness to learn and actual or potential barriers to learning. Integrates primary and secondary diagnosis into the educational plan. Documents assessment / reassessment, plan, interventions and outcomes according to policy. Communicates and documents change in-patients condition to all appropriate individuals. Documents emergency interventions according to policy. Documents medication use and controlled substance per policy Acts as a patient advocate and uphold patient’s rights. Ability to function and handle emergency situations in a fast-paced environment. Initiate dysrhythmia monitoring. Supervise cardiac rehab sessions, monitor EKG, update exercise prescription, and document patient response to exercise. Conducts medication reconciliation for all patients in CardioPulm Rehab per policy Maintains ongoing communication with physicians by writing program reports and discharge summaries.
