Job Description :
We are currently seeking a dedicated and experienced LVN / LPN to join our team. In this role, you will play a vital role in supporting the administrative and clinical operations of our agency, ensuring the smooth delivery of care to our patients.
Responsibilities :
Serve as the primary point of contact for patients, families, and caregivers, providing exceptional customer service and addressing inquiries or concerns promptly.
Patient visits, coordinate transportation, and communicate with field staff to ensure timely and efficient delivery of care.
Assist with the intake process for new patients, including verifying insurance coverage, obtaining necessary documentation, and coordinating initial assessments.
Maintain accurate and up-to-date patient records, including electronic health records (EHR), documentation of visits, and billing information.
Collaborate with the healthcare team to develop and implement comprehensive care plans, ensuring that patient needs are met and care is coordinated effectively.
Assist with quality assurance activities, including audits, chart reviews, and performance improvement initiatives.
Provide support to clinicians and other office staff as needed, including answering phones, filing paperwork, and assisting with special projects.
Qualifications :
Current LVN / LPN license
Minimum of 1 year of experience in clinical nursing, preferably in home health or hospice setting.
Strong organizational and multitasking abilities, with excellent attention to detail.
Proficiency in computer skills, including Microsoft Office and electronic health records (EHR) systems.
Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, and healthcare professionals.
Ability to work independently and as part of a team in a fast-paced office environment.
