**Job Summary:**
The Per Diem Hospice Nurse Practitioner provides advanced nursing care and support to patients with terminal illnesses and their families in various settings, including private residences, hospice facilities, and nursing homes. This position offers flexibility in scheduling, allowing Nurse Practitioners to work on an as-needed basis.
**Responsibilities:**
1. **Patient Assessment and Management:**
- Conduct comprehensive assessments of patients with advanced illness, including physical, psychosocial, and spiritual dimensions.
- Develop and implement individualized care plans in collaboration with the interdisciplinary hospice team to address symptom management, pain control, and psychosocial support needs.
2. **Medication Management:**
- Prescribe and adjust medications as necessary to optimize symptom control and comfort for patients receiving hospice care.
- Educate patients and caregivers on medication management, potential side effects, and strategies for symptom relief.
3. **Palliative Care Consultation:**
- Provide expert consultation on palliative care principles and practices to patients, families, and healthcare team members.
- Collaborate with attending physicians, nurses, social workers, chaplains, and other hospice team members to deliver holistic care and support.
4. **Advance Care Planning:**
- Facilitate discussions about advance directives, goals of care, and end-of-life preferences with patients and families.
- Assist with the completion of advance directives and ensure alignment with patients' values and wishes.
5. **Family Education and Support:**
- Offer education and support to family members and caregivers on the hospice philosophy, caregiving strategies, and coping mechanisms during the end-of-life journey.
- Provide emotional support and counseling to address grief, loss, and bereavement needs.
6. **Documentation and Reporting:**
- Maintain accurate and timely documentation of all patient assessments, interventions, and care plans in electronic health record (EHR) systems and hospice-specific databases.
- Complete required documentation for billing, compliance, and quality assurance purposes.
**Qualifications:**
Current licensure as a Nurse Practitioner (NP) in the state of practice, with specialization in adult-gerontology, family, or palliative care.- Certification as a Nurse Practitioner by an accredited certifying body (e.g., AANP, ANCC).
- Minimum of 2 years of experience in hospice, palliative care, or a related field.
- Strong clinical assessment and management skills, with expertise in symptom management and end-of-life care.
- Excellent communication and interpersonal skills, with the ability to provide compassionate care and support to patients and families.
- Proficiency in electronic health record (EHR) documentation and Microsoft Office applications.
**Working Conditions:**
Per diem scheduling allows for flexibility in hours and shifts, based on the needs of the hospice organization and availability of the Nurse Practitioner.- Work settings may include patient homes, hospice facilities, nursing homes, hospitals, and other healthcare settings.
- Some evening, weekend, and holiday shifts may be required based on patient needs and staffing requirements.
