What are the responsibilities and job description for the Licensed Clinical Social Worker position at PACE HEALTH NETWORK, LLC?
PACE Health Network, LLC is an outpatient mental health practice based out of Whiteville NC. All patients are seen in long term care facilities (ie assisted living and nursing facilities). Staff of PACE Health Network have been providing services in facilities for 26 years, and 4 of the facilities have been with us since start-up.
PACE Health Network, LLC is a private practice that provides high-quality, personalized outpatient services to individuals who live in assisted living and nursing facilities throughout southeastern North Carolina. PACE staff is dedicated to providing a full range of mental health services addressing the needs of the patients, families, and the staff to optimize, enhance, and promote the quality of life of the persons in the facility. Outpatient psychotherapy services are provided by Licensed Clinical Social Workers (LCSW) and Licensed Clinical Mental Health Counselors (LCMHC) and medication management is provided by Psych Mental Health Licensed Nurse Practitioners. It should be noted that this practice only offers outpatient therapy and medication management, but not crisis management teams.
PACE Health Network licensed therapist are usually present in the facilities at least once a week providing individual counseling sessions for the patients in the facilities. Treatment may focus on adjustment issues, depression, anxiety, behavioral problems, and/or other mental health problems, including management of chronic psychiatric conditions. Therapist are hired as contract labors and are compensated based on a percentage of reimbursements. The position is flexible with creating your own schedule within parameters of a work week. Commuting to the facility is required. PACE provides credentialing with Medicare and other third party insurers, provides a billing agent for timely processing of claims, and enrollment in our EMR system for documentation of weekly progress notes, clinical assessments, and treatment plans. Therapist are also encouraged to consult with our medication management providers to ensure best treatment practices. Also to sustain high-quality services, therapist need to maintain good rapport with the staff of the facilities that we serve.
Requirements/Responsibilities:
· Licensed Clinical Social Workers (LCSW) or Licensed Clinical Mental Health Counselors (LCMHC)
· Driving to the assisted living or nursing facilities
· Independent Malpractice Insurance policy
· Contract Labor/1099 Employee (self-motivated, flexible hours, tax benefits)
· Good Facility Relationships - provide stability for the facilities we serve by consistently being present in the building weekly and offering support to staff and/or families. Our consistency (i.e. being in the facility on the same day each week) establishes our dependability and the facility’s trust in us. The facility trusts what they can predict. We need to be predictable.
· Therapy services should be provided between the hours of 8:00 to 5:00 Monday through Friday. Exceptions can be made but need to be discussed
· Upon arrival and at the end of the day, check with the Administrator/Office Manager/Director of Resident Services about any concerns, problems, issues, or events that have occurred with the patients since your last visit. Please share with staff, within the limits of confidentiality, concerns of the patients.
· Documentation of services in our EMR system within a “reasonable amount of time”. That documentation includes Clinical Assessment (CCA), Treatment Plan (TP), and Progress Notes (PN).
· Psychotropic medication issues reported to you by patient or staff need to be discussed with the facility liaison before the close of the day in which they were discussed with you.
· PACE provides you facilities/patients (insurances verified), enrollment in our EMR system for documentation, credentialing with Medicare and other third party insurers, and a billing agent for timely processing of claims.
Salary : $50,000 - $85,000
