We are seeking a Registered Nurse (RN) to serve as a Care Manager Supervisor at PACE North. This vital leadership role is responsible for providing participant care coordination, supervising a team of dedicated staff including RNs and Social Workers, and ensuring the delivery of high-quality services to our participants. As a key member of our leadership team, you will provide guidance and ensure continuous improvements in care delivery while also advancing your own professional growth.
Key Responsibilities :
- Lead and supervise assigned staff, ensuring effective recruitment, training, performance management, and professional development.
- Foster a collaborative and positive environment, encouraging teamwork and communication across all departments.
- Ensure accurate and timely documentation of services provided, including home visits and facility-based care.
- Coordinate care plans and assist in participant admissions, discharge planning, and care management.
- Oversee the RN on-call rotation, ensuring after-hours medical assistance is provided as needed.
- Engage with participants, families, and interdisciplinary teams to evaluate and enhance participant care.
- Support quality initiatives, including audits and performance metric monitoring to meet organizational standards.
- Ensure compliance with participant rights and ensure quality care practices are upheld.
