What You'll Do:
As a vital member of the Care Management Team, you'll be on the frontlines of providing critical healthcare support and administrative assistance. Your role will include:
- Patient Care: Conducting home visits and providing treatments under the guidance of our RN Care Team, including phlebotomy, wound care, toenail care, and more. This will involve regular daytime travel throughout our seven county service area. (We provide reimbursement for mileage!)
- Documentation & Administrative Support: Documenting all patient interactions and care delivered using Electronic Medical Records (EMR) to ensure accurate, detailed reporting.
- Coordination: Ordering and tracking services, ensuring timely delivery of care, and handling necessary paperwork for smooth transitions between healthcare providers.
- Team Collaboration: Working alongside an incredible team of healthcare professionals, supporting each other to deliver the highest quality care to our participants.
What You’ll Bring:
To excel in this role, you'll need a combination of experience, skills, and passion:
- Experience: Preferably 6 months of clinical experience (Medical Assistant, Nursing Assistant, or Nurse Aide), along with experience working with the frail or elderly population.
- Certifications: CPR certification is required (we can train!). Medical Assistant Certification is highly preferred.
- Skills: You are a proactive, independent thinker with strong communication skills, medical terminology knowledge, and clerical abilities. You can work well in a team environment and handle a variety of responsibilities with ease.
- Physical Stamina: You’re ready to stay active with frequent standing, walking, lifting, and bending – always keeping safety and care at the forefront.
