The Home Care Supervisor (RN) oversees the delivery of home care services for participants in the PACE program and plays a critical role in maintaining the overall quality and effectiveness of services provided to PACE s participants. This role involves collaborating with the Interdisciplinary Team (IDT), participants, and their families and / or caregivers to develop personalized care plans. The Home Care Supervisor (RN) will also manage a team of Registered Nurses, Licensed Practical Nurses, and Medical Assistants, ensuring appropriate care is delivered with the highest standards. This is a full-time, on-site opportunity. The typical schedule is Monday through Friday, 8 : 00 a.m. to 4 : 30 p.m., but it is subject to modification based on the department s needs. On-call duty is to be expected. Some travel is required.

Requirements :
Education : An Associate s degree in nursing is required.
Experience : At least two years of nursing experience, preferably in-home health care or community health. At least two years of supervisory experience is required. Minimum of one year of experience working with the frail elderly population in home care, community health, or nursing homes.
Number and Type of Employees Supervised : 6-10. Nurses and Medical Assistants.
Licensure, Registry, or Certification Required : Must possess an active Michigan RN license. Proof of license must be provided during the onboarding process. A Valid MI driver s license is required in addition to proof of current car insurance. Current CPR certification by the American Heart Association is required, or willingness to obtain certification as offered by PACE of Southwest Michigan.
Special Training : Only acts within the scope of his or her authority to practice. Meet a standardized set of orientation and / or individual competencies for the specific position description established by PACE of Southwest Michigan and approved by CMS before working independently.
Immunizations : Be medically cleared for communicable diseases. Immunizations must be up-to-date before engaging in direct participant contact.
Specific Requirements and Considerations : May be subject to exposure and handling of infectious waste, diseases, conditions, and / or hazardous chemicals, etc., including potential exposure to TB, AIDS, and / or Hepatitis B.

Key Responsibilities :
Oversee the appropriateness of home and community-based services provided to the participants by PACE, the utilization of related resources, and the quality of care provided.
Participates in the development care planning with the Interdisciplinary Team, and the participant and their family or caregiver.
Coordinates and provides oversight of 24-hour care delivery and the implementation of all home care services which include PACE or contracted nursing and personal care services to ensure that quality services are provided to meet participant needs in all home and community-based settings per the care plan.
Coordinate and manage the scheduling of home care Registered Nurses, Licensed Practical Nurses, and Medical Assistants. Modify schedules as needed and communicate with the appropriate groups.
Monitors and audits the documentation of home care Registered Nurses, Licensed Practical Nurses, and Medical Assistants on a consistent basis.
Ensure staff monitors the physical and mental well-being of the participant and their caregiver(s) according to the care plan and follows up appropriately.
Monitor changes in home care orders and communicate changes to appropriate internal staff and external providers.
Provides oversight to ensure DME needs are met.
Performs regular supervisory visits of PACE home care nurses and aides, as well as contract home care agents to ensure the provision of quality care.
Ensures ongoing adherence to the home care guidelines and the completion of required training by home care staff.
Adheres to HR policy and procedures related to hiring, coaching, and managing direct reports.
Reconciles and audits all contract homecare billing to authorized hours and services.
Provides participant / caregiver education as needed.
Participates in IDT meetings, Care conferences, and team huddles regularly.
Demonstrates active participation in continuous process improvement and quality initiatives.
Provides in-home care for participants on an as-needed basis.
Ability to critically assess the physical, psychosocial, and home care needs during initial, periodic, routine, and acute assessments conducted minimally every 6 months.
Performs weekday and weekend on-call duties.
Abides by all established PACE of Southwest Michigan policies, rules, and regulations, as well as federal and state regulations that govern the operations of a PACE organization.
