Job Title: Community Relations and Enrollment Liaison
Supervisor: Director of Community Engagement
FLSA Status: Non-exempt; Full-Time
Job Summary: Under the general supervision of the Director of Community Engagement, the Community Relations and Enrollment Liaison is responsible for providing outreach and education, developing collaborative partnerships, establishing provider contracts and coordinating enrollments.
Tasks and Time: Outreach and education-40%; Enrollment-20%; Contract coordination-20%, Administrative-10%.
Education and Special Training: Bachelor's degree or equivalent experience.
Experience: Minimum of three years of professional experience in health care marketing, sales, communication or human services.
Licensure, Registry or Certification Required: Must have a valid NC driver's license and vehicle. Position requires 50% or more travel within specified service areas.
Knowledge, Skills, and Abilities: Must be able to work effectively in a team environment; Must possess excellent communication and public speaking skills. Expert level experience in Microsoft Office (Word, Excel, PowerPoint, etc.,) Knowledge of Medicare and Medicaid desired.

Essential Job Functions:
- Effectively communicates PACE's philosophy, mission, and services to ensure understanding of the PACE model of care.
- Assists with the development, implementation and evaluation of the overall marketing strategy for the recruitment and retention of PACE participants to build and maintain an optimal census.
- Establish contract agreements for services (specialists, home care, home health, etc.). Provides contractor orientation (new and annual) and maintains accurate and organized records.
- Plans and implements outreach programs and networking events in the facility and the community, based on strategic plan and territory, as assigned. Connects with community leaders, legislators, health care professionals and others to develop and maintain relationships and/or collaborative partnerships.
- Conducts tours of PACE facility and provides in-person and telephone consultations to potential participants and their families and/or caregivers to include resource information and education materials. Makes referrals to other agencies as appropriate.
- Evaluates potential participants according to program guidelines, conducts assessments and completes and submits documentation for enrollment including safety evaluations and Medicaid forms.
- Documents presentations/visits/calls using CRM or Excel. Provides weekly, monthly, quarterly and annual statistical reports.
- Creates and distributes monthly electronic and/or printed publication for professional referral sources, community partners and contracted providers.
- Attends and participates in staff meetings, departmental meetings (Intake, Interdisciplinary Team) and other related meetings and activities.
- Other duties as assigned.
