Job Summary

Under the supervision of the CEO, the Director of Clinical Services has authority and responsibility for the development, leadership, and supervision of Center Managers, Clinic Supervisors and Clinical Administrative Assistants for PACE of Guilford and Rockingham Counties, Inc d/b/a/ PACE of the Triad locations. The Director of Clinical Services will ensure that decisions regarding the medical, social, physical, and emotional support services are driven by the Interdisciplinary (IDT) Team process and supports PACE of the Triad’s Mission, Vision, and Values. In the absence of the Director of Operations, the Director of Clinical Services also serves as back up and works closely with the Director of Operations to ensure compliance with operational guidelines, and organizational processes, workflows, and systems. This position facilitates IDT meetings and works closely with Medical Director and Director of Quality and Compliance to collect and analyze quality data. The Director of Clinical Services ensures that Center Managers monitors compliance with PACE regulations and applicable state and local regulations. This position ensures ongoing clinical education and training of applicable staff and has responsibility for 24-hour care coordination.


Specifications


Education and Special Training: Bachelor of Science in Nursing required. Master’s degree preferred.


Experience: Five years’ experience in health care management or nursing with at least three years of leadership experience. Minimum of one year experience caring for the frail or elderly populations. Preferable experience in Home Care or Long-Term Care, Acute Care, and/or Managed Care Settings. Ideally PACE program experience.


Licensure, Registry or Certification Required: Licensed as a Registered Nurse in North Carolina (NC), Valid NC driver’s license and vehicle.


Knowledge, Skills, and Abilities: Knowledge of holistic care in the geriatric population. Able to inspire others through change and adversities. Must thrive in a team environment. Possess excellent interpersonal, organizational, group facilitation and consensus-building skills. Must also possess other skills necessary to provide care for geriatric patients. Proven leadership, management, and supervisory skills. Able to learn, teach and research, as well as solve problems and think analytically/critically. Strong oral and written communication skills required. Ability to handle difficult and unusual interpersonal situations effectively. Only act within the scope of his or her authority to practice. Must meet a standardized set of competencies for the specific position description established by PACE of the Triad and approved by Center for Medicare and Medicaid Services before working independently. This position must have the ability to drive the successful achievement of goals of the organization and be instrumental in leading efforts for program growth, expansion, efficiency, and continuity of operational practices across multiple PACE Centers.

With or without reasonable accommodations must have the ability to: bend and/or stoop frequently; have independent mobility in order to climb stairs and walk distances; lift and position participants of all weights and heights utilizing correct body mechanics; lift and carry pieces of durable medical equipment; lift and transport at least 50 pounds during the course of providing care, treatment and or services; read, write and speak in English; see medical records and various documents clearly; use various forms of communication including but not limited to telephone, computer, faxes etc.; hear blood pressures and spoken communication; perform repetitive hand and wrist movements.


Key Responsibilities


Leadership:

  • Serves as an effective member of the Senior Leadership Team who works collaboratively to support the mission, vision, and values of the organization.
  • Upholds the core values of the organization and demonstrates the “do no harm” concept to teammates to build trust and effective working relationships.
  • Provides oversight of daily care of participants through Center Managers including, but not limited to, appropriate utilization of resources. Assists with oversite of clinical budget expenses.
  • Directly supervises the Center Managers, Administrative Assistant, and other staff as designated.
  • Completes Rounding for Outcomes sessions with each direct report to assure effective communication between CEO and direct reports and other staff as needed.
  • Serves as facilitator of or monitor of IDT care planning. Also serves as facilitator of family meetings and team meetings as needed.
  • Provides oversite and accountability of Center Mangers to assure coordination of care between teams and other areas of program operations.
  • Arranges for or serves as back up for the Center Manager positions in their absence.
  • Supports PACE of the Triad’s mission of assisting frail, older adults to remain in the community by avoiding unnecessary hospitalizations and premature nursing home placement.
  • Provides human resources guidance to the Center Managers and other staff as designated.
  • Attends and participates in Leadership meetings and staff meetings.
  • Member of Quality Improvement and Compliance committees and is instrumental in driving performance improvement initiatives as well as participant satisfaction initiatives.
  • Collaborates with the Medical Director in the development and implementation of clinical practice guidelines and standards of care for participants enrolled in PACE of the Triad.
  • Works effectively with Director of Operations, Medical Director, Director of Intake and Outreach, as well as Director of Quality to ensure consistent communication regarding clinical care and operational efficiency.
  • Assist Human Resources with Recruiting, Hiring, Orienting and ongoing Training of staff to meet demands based on participant care needs.
  • Provides routine meetings with direct reports to discuss topics of concern or interest.
  • Supports the IDT and other staff to promote unity among team members while interacting with the team, other co-workers, and/or participants and families. Accepts teammates’ differences in professional assessment findings, as they relate to participant care, and supports teams’ decisions while promoting a united team for the participants’ continuity of care.
  • Possess competency in interpreting local, state, and federal regulations to assure staff understanding and knowledge.
  • Monitors direct reports, and other designated staff as needed, regarding regulatory guidelines to assure compliance.
  • Implements infection control policies and procedures and monitors preventative measures by tracking and trending for infection rates and implements appropriate measures.
  • Ensures 24-hour care coordination by assigning enrollees to teams and assigning staff for on-call.
  • Other duties as assigned.

Environmental Factors

Tasks with potential for exposure to blood borne pathogens include, but are not limited to the following:

  • Assisting with mouth care
  • Assisting with care of non-intact skin
  • Assisting incontinent participants with personal care
  • Assisting with procedures in the clinic

Demonstrates knowledge of and supports PACE mission, vision, values, standards, policies and procedures, operating instructions, confidentiality standards, and the code of ethical behavior.
