Job Title: Social Work/Case Manager (MSW)

Job Summary
Responsible for the direct social work case management service to the participants of the PACE of Guilford and Rockingham Counties program. Provides screening, psychosocial assessment, care plan implementation and appropriate interventions including but not limited to counseling, referrals, and education.
Specifications
Education: Master’s Degree from an accredited School of Social Work
Experience and Special Training: Two years’ experience in a health-related area. Minimum of 1 year experience working with the frail or elderly population
Licensure, Registry or Certification Required: Valid NC driver’s license and vehicle.
Knowledge, Skills, and Abilities: Only act within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by PACE of Guilford and Rockingham Counties and approved by Centers for Medicare and Medicaid Services (CMS) before working independently. Individual, family and group therapy skills for geriatric participants and their families, analytical thinking and problem-solving abilities. With or without reasonable accommodations: Ability to bend and or stoop frequently. Have independent mobility in order to climb stairs and walk distances. Utilize correct body mechanics. Read, write and speak in English. Communicate effectively both orally and in writing. Ability to perform repetitive hand/wrist movements.
Key Responsibilities
- Participates in the Interdisciplinary Team’s (IDT) initial assessments, care planning and periodic reassessments (minimally every 6 months) of participant’s case. Assessments are to be completed prior to the scheduled team meeting. Communicates participant changes to team members.
- Conducts home visits to private homes of PACE participants, frequent travel in Guilford and Rockingham Counties.
- Maintains current written case management records, including periodic reassessment of program participants.
- Coordinates 24-hour care delivery.
- Provides individual and family counseling, develops and leads group counseling and activities.
- Refers participants and families to appropriate community agencies or facilities, acts as liaison with such organizations and as advocate for participants.
- Encourages effective relationships among staff geared to team building and maintenance of a cohesive team.
- Consults with and advises staff members as to the relationship of social, emotional and cultural factors to health and medical care, and as to the availability of social services in the community.
- Participates in Program and Policy development of the Social Work component.
- Documents participant changes appropriately in the medical record.
- Participates as an active member of the IDT and in participant care planning.
- Attend staff meetings.
- Other duties as assigned.
