Job Summary:
The Activities Coordinator for the PACE (Program of All-Inclusive Care for the Elderly) program is responsible for planning, organizing, and implementing engaging activities tailored to the needs of elderly participants. This position plays a crucial role in enhancing the social, physical, emotional, and cognitive well-being of participants while fostering a positive and inclusive environment.
Key Responsibilities:
- Develop, coordinate, and implement a variety of recreational, educational, and social activities to meet the needs and interests of PACE participants.
- Assess participant preferences, abilities, and needs to create individualized and group activity plans.
- Ensure all activities support participants' functional abilities and promote overall well-being.
- Organize special events, outings, and holiday celebrations in compliance with program policies and safety regulations.
- Collaborate with interdisciplinary team members, including nurses, therapists, and social workers, to integrate activities into participants’ care plans.
- Maintain accurate documentation of activities, participation levels, and progress reports.
- Assist in transporting participants to and from activities, ensuring safety and engagement.
- Supervise volunteers and staff assisting with activities, providing guidance and support as needed.
- Manage activity supplies, budget, and equipment, ensuring resources are available and well-maintained.
- Stay current with best practices in senior recreation and implement innovative programs to enhance participant engagement.
- Additional responsibilities may be assigned.
Qualifications:
- High school diploma or equivalent required; Bachelor's degree in Recreation Therapy, Gerontology, Social Work, or a related field preferred.
- Minimum of 1-2 years of experience in activity coordination, preferably in a healthcare, senior care, or PACE program setting.
- Strong organizational, communication, and interpersonal skills.
- Ability to work effectively with elderly individuals, including those with cognitive impairments or mobility challenges.
- Knowledge of recreational therapy techniques and best practices.
- Ability to adapt activities to diverse physical and cognitive abilities.
- CPR and First Aid certification (or willingness to obtain).
- Valid driver’s license and reliable transportation preferred.
- Proficiency in Microsoft Office Suite and ability to document electronically.
Physical and Working Conditions:
- Ability to stand, walk, bend, and lift up to 25 pounds.
- Work is performed in an indoor/outdoor setting, with occasional outings requiring travel.
