Title - Care Manager RN
Duration - 8 months with possible extension
Shift : 8 am 5 pm, M-F
Location - Dayton OH
Pay rate - $40-48.00/hr
Description
- Required to do field visits (member-facing)
- Two different types of case loads (one working with over 60 waiver population; other will be working with the under 60 population)
- Assessments face-to-face visits for maintaining safety welfare; audit their caseloads
Working Culture
- HP/BU: Buckeye
- Team Culture: 13 Case Managers (small team), but always willing to jump in to help each other out when the need arises; sets up team call (weekly huddles) should any issues arise when providing care for their members
- Key Projects: N/A, not now
- Purpose of the team: implement a 3-way contract that exists with ODM, CMS, and Buckeye. Our entire existence is because this 3-way contract exists.
Typical Day to Day
- Schedule: 8a-5p, M-F (NO OT expectations)
- Task Breakdown: We work out of EMR system has a tasking system where we prioritize our day to day
- Interaction level w/ team: Group chats daily
- Work environment description: Remote so MUST be self-starter

Requirements:
Education/Certification Required:
- Associate's degree of Nursing
Licensure :
- RN (OH or compact)
Years of experience required-
- Behavioral Health and experience with working with mental health and substance abuse
- Years of experience required- For RN, must have Home Health background
- Disqualifiers- only having hospital experience
- Best vs. average- Someone who already has waiver knowledge vs someone who only has home health background
- Performance indicators- documentation requirements; run weekly reports to ensure visits are completed and documentation is completed timely
Top 3 must-have hard skills-
- MUST have technology skills as we use multiple systems; MUST be a self-starter; and great time management and prioritization
- Level of experience with each- MUST be proficient with technology
- Stack-ranked by importance- Being a self-starter and great with time management, you should automatically know where to start and navigate as this is a constant-changing environment
- Candidate Review & Selection- no upcoming time off; we typically interview (2-panel), but ultimately, my approval only is needed for hiring
