- Compensation for this role pays up to $66,560.00-$87,726.30. Compensation will vary based on qualifications, skills, experience, and geographic location. The salary range may vary if you reside in a different location or state than the location posted.

JOB SUMMARY
Works within the values and clinical philosophy of the agency. Under the general supervision supports Program Managers-Managed Care and Program Supervisors-Managed Care by ensuring quality comprehensive care management assessments (including risk, social determinants of health) and plans in managed care programs consistent with the requirements of CalAIM.
RESPONSIBILITIES AND DUTIES
- Apply advanced skills to gather assessment information in development of the care plan and applies information and resources to ensure quality care.
- Understands and applies multi-modal approaches and perspectives to whole-person care.
- Actively participates in the identification of team opportunities for improvement or identification of deficiencies and in the development of strategies to address or bridge gaps.
- Participates in Quality Improvement (CQI) activities and project implementation.
- Performs other related responsibilities, as assigned, to support specific department/business needs.
QUALIFICATIONS

Minimum Education and/or Experience
- Master's degree in social work or a closely related field (e.g., Marriage, Child and Family Counseling or Psychology) required.
- Three (3) years of supervised clinical experience.
- Licensed according to California state law (Welfare and Institutions Code section 5600.2)
PREFERRED QUALIFICATIONS
- Minimum of 2 years of clinical experience with complex care
- Experience with care management plans.
- Knowledge of Social Determinants of Health and Crisis management experience preferred.
HIGHLY DESIRABLE SKILLS
- Experience, knowledge, skills, and abilities with culturally diverse populations.
- Speak, read, and/or write another language.
