Pacific Clinics serves children, transitional age youth, families, adults, and older adults. We offer a full range of mental and behavioral health services, foster care and social services, housing, continuing adult education and early childhood education programs to Medi-Cal eligible individuals and families. POSITION SUMMARY:
Under the general supervision of a Director/Team Leader the Community Health Worker is an individual who has experience with the health and mental health system and who has been trained to provide support, self-help services, presentations on behavioral health prevention and wellness topics to community members, community organizations, schools, faith based organizations, and first responders. The community health worker will also promote mental health awareness, education, and available resources specifically to the unserved, underserved and inappropriately served communities.
ESSENTIAL DUTIES AND RESPONSIBILITES:
• Under direct supervision, provides supportive assistance to, and the modeling of self-advocacy with targeted community members to help them research and access needed and desired community resources.
• Works cooperatively with others to assure the smooth and seamless delivery of comprehensive services to members.
• Under direct supervision, completes documentation and other paperwork in a timely and accurate manner, and in accordance with defined standards and funding source requirements.
• Promotes health, mental health, wellness, and resources specifically to unserved, underserved, and inappropriately served communities.
• Provides education on substance abuse, mental illness prevention, and wellness topics.
• Conducts educational presentations to include but not limited to faith-based groups, community groups and school groups (such as Parent Teacher Association PTA)
• Travels to family homes in the community to do “Knock and Talk” sessions in targeted neighborhoods to provide outreach, education and support as well as for presentations, outreach activities, and meetings.
• Conducts one-on-one or small groups in homes or other appropriate community settings
• Provides information at community cultural events and fairs
• Meets with local agencies to advocate for policy change that can include, but is not limited to, primary care facilities, government agencies, and local businesses.
• Advocates for the appropriate communication and receipt of services requested and required on behalf of the consumer with medical systems, behavioral health systems, and social service systems.
• Assists responders to recognize and respond effectively to early signs of mental illness, provide health promotion, educational services, alternative activities, or identify factors than can contribute to the development of a behavioral health condition.
• Provides outreach on signs and symptoms of mental illness and educational promotion of wellness topics to community establishments and identified unserved/underserved community members
• The Community Health Worker will serve as a liaison to mental health resources and programs in the community
• Crisis Intervention-Must be able to assess the level of crisis and provide non-therapeutic crisis intervention and initiate referrals to proper agencies for treatment. Shall provide services that assist participant(s) in accessing needed medical, educational, social, prevocational, vocational, rehabilitative, or other community services.
EDUCATION and/or EXPERIENCE/POSITION REQUIREMENTS:
• High school diploma or GED required.
• Lived experience with mental illness or the mental health system preferred.
• Must possess a valid California driver's license and maintain an insurable driving record under the Clinics' liability policy.
• Must know or quickly acquire basic computer/word processing skills.
• Flexible work schedule to include occasional evenings, weekends and holidays.
• Must know or quickly acquire extensive knowledge of local community and medical resources.
• Demonstrated ability to work in Windows environment (including Word and Excel).
• Must possess a valid CA driver's license and insurance with two years experience and maintain an insurable driving record under the agency’s liability policy.
