Pacific Crest Hospice of Ventura provides professional medical care and volunteer support to people with life limiting illness. Our goal is to provide excellent care so that our patients remain comfortable in their own homes and stay as independent as possible.
Pacific Crest Hospice of Ventura is recruiting experienced and compassionate hospice professionals and is currently accepting applications for an LVN for on-call shifts.
General Responsibilities:
- Administers nursing care to terminally ill patients under the direction and supervision of a Registered Nurse (RN)
- Observes signs and symptoms and reports to physician and RN reactions to treatments, including medications, and changes in patient's physical or emotional condition
- Teaches and counsels patient and caregivers regarding patient nursing care needs
- Triaging after-hour calls, completing visits as needed.
- Care provided in patient homes, RCFEs (assisted livings and board and cares), nursing homes
- Other duties as assigned
Ideal Candidate Skills/Knowledge:
· Graduate for accredited school of nursing and current California LVN license (required)
· Minimum 1 year of experience as an LVN (required)
· Hospice or Home Health experience (preferred)
· Valid California Driver's License and reliable transportation
