Job Title: Disaster Case Manager

Department: Immigration Services

Reports to: DCMP Supervisor/Dep. Director, Social Services

Summary: The Disaster Case Manager (DCM) works directly with individuals and households affected by the August 2023 Maui wildfires, acting as a facilitator and advocate to empower their recovery process. The DCM helps Limited English Proficient (LEP) impacted families identify unmet needs and determine recovery goals, then collaborates with other agencies to meet those needs and goals, while ensuring the family is empowered to lead their own journey to recovery. A team of five Bilingual DCMs report to the Disaster Case Management Supervisor. DCMs are full-time, long-term positions and candidates.

The mission of Pacific Gateway Center is to empower low-income residents, immigrants, refugees and other vulnerable populations to achieve self-sufficiency through skill-building and access to opportunities while respecting cultural heritages. On Maui, we provide immigration legal and social Bilingual Case Management services through our satellite office, the Maui Immigrant Resource Center, located at the J. Walter Cameron Center.

JOB RESPONSIBILITIES

  • Identify clients’ unmet disaster-related needs in areas such as housing, financial support, employment, physical health, mental health, child care, etc.
  • Complete all steps of the case management procedure, including intake, case management, service plan creation and exit procedures, for each client and family.
  • Facilitate trauma-informed and crisis support case management sessions with families.
  • Provide resources, referrals, and workshops related to survivors’ unmet needs.
  • Serve as an advocate for families with community service agencies.
  • Coordinate with partner organizations while maintaining and advancing PGC’s resources
  • Maintain case file records for every family, including documenting all case management activities and services provided, in physical files and the case management software.
  • Complete exit procedure including exit forms and exit survey.
  • Attend training sessions to learn to identify, validate, and collaborate on solutions to meet survivor needs.
  • Attend all mandatory training provided by Program Administrator(s).
  • Additional responsibilities as needed.

Required Qualifications & Experience

  • Required, 4-year degree in human services field or equivalent experience.
  • Required, fluency, or have strong conversational skills, in Spanish, Tongan, Ilocano/Tagalog, Pohnpeian or Marshallese
  • Self-motivated and passionate about supporting the recovery efforts on Maui.
  • Empathetic and able to empower families’ choices.
  • Detail-oriented, with solid communication and presentation skills.
  • Team player who works well with a variety of personalities.
  • Flexible with work schedule, able to accept feedback and adaptable to change.
  • Currently residing on Maui with a driver’s license and reliable transportation.
  • Preferred prior professional experience working in the public interest field.
  • Other duties as necessary.

COMPENSATION/HOURS

  • $54,080 - $62,400 annually commensurate with experience;
  • Full-time; 40 hours per week. Monday – Friday; flexible with some weekend and evenings as required substitutes to meet client needs
