Position Summary:
Coordinates the delivery of rehabilitation services to patients/residents in collaboration with the facility’s care staff. Continually monitors and documents the cost-effectiveness of treatments provided, facilitating admission and discharge processes, serving as patient advocate and liaison to insurance carriers and other medical management professionals.

- Obtain an LVN or RN license
- Prior experience in skilled nursing preferred but not required (if not, have you work in a field related to case management?)
- Outstanding communication (oral and written) skills
- Ability to multi-task and treat everyone at work kindly and professionally
- Works collaboratively and independently, following procedures and guidelines, making recommendations for improvements
- Completes tasks on time, reporting up on successes, challenges, and offering solutions to problems
- Emotional maturity – ability to remain calm and professional under stress
Responsibilities:
- Greet new patients/residents ensuing a positive admission experience.
- Evaluate and communicate resident status, changes in functions and care plans to payers
- Document all payer interactions regarding resident progress, expected outcomes and special instructions
- Meet with interdisciplinary team to coordinate services ensuring resident’s total regimen of care is maintained
- Coordinate and communicate with business office and medical records to ensure accurate census and payment of managed care and Medicare residents
