PACT MSO has busy Endocrinology office seeking a Certified Diabetes Educator. This position is for either an RN with a discipline as Registered Dietician, a Advanced Practice Registered Nurse (APRN) or a Physician Assistant (PA). The hours are Monday through Thursday from 8 : 00am to 6 : 30pm. This position requires traveling between Milford and Hamden locations and will follow the provider to each location.
Summary
This position enters data into any and all databases including but not limited to EMR, vendors, carriers, etc.
Essential Functions
- Develop personalized care plans for patients with Diabetes
- Provide instruction on self-monitoring of blood glucose levels
- Teach patients how to administer insulin and other diabetes medications
- Offer nutritional counseling and meal planning guidance
- Educate patients about the importance of physical activity and exercise
- Assess patients' knowledge and skills related to diabetes management
- Utilizing EPIC in full capacity of patient care
- Support patients in setting and achieving diabetes-related goals
- Monitor and documents patients progress over time
- Stay up-to-date with the latest diabetes management guidelines and research
- Provide resources and support for managing diabetes-related complications
- Identify gaps in diabetes knowledge to address and work to fully equip the patient with skill sets needed for self-care.
- Provide full diabetes self-management training to patients by using digital tools and virtual interactive platforms over a series of visits.
- Support and educate patients on the use of diabetes technology : including proprietary blood glucose meters, continuous glucose monitors and diabetes smartphone apps.
- Work with medical provider to explore other interventions or escalate therapy as needed.
- Identify SDOH (Social determinates of Health) and apply problem solving skills to ensure optimal outcome by coordinating referrals to community resource programs, such as rehabilitation, financial assistance, behavioral health and other social services when needed.
- Serve as a resource to answer any clinical diabetes questions from the team and provide ongoing collaborative diabetes education to internal care team members.
- Using integrated remote patient data, from Glooko for medication adjustment. Use validated titration protocols with support from the medical provider when needed.
