Job Description: As an RN Case Manager, you'll take the lead in delivering top-tier patient care in our home health agency, licensed by the California Department of Public Health. You'll conduct patient assessments, create personalized care plans, and oversee the work of Home Health Aides/CNAs and LVNs, ensuring seamless, high-quality care. You'll collaborate with interdisciplinary teams, make a direct impact on patient outcomes, and build strong relationships with patients and their families. RESPONSIBILITIES:
- Demonstrates support for the agency’s core values to promote quality, efficient, comprehensive, and effective patient service.
- Participates in and supports the agency's quality assessment and performance improvement activities.
- Participates in orientation, competency testing, and in-services of all new personnel as needed.
- Oversees ongoing clinical training of care staff, ensuring nurses and care staff are competent to care for each client to which they are assigned.
- Supports coordination of home health care services with all disciplines, Director or Supervisor, as requested.
- Performs the initial comprehensive and ongoing assessments on each assigned patient’s physical, functional, psychosocial, environmental, and activities of daily living. Initiates preventive and rehabilitative nursing procedures.
- Develops and revises the plan of care with the patient and physician, as needed, and ensures all Plans of Care are updated and completed.
- Provides direct care, treatments, assessments, and education to patients and their families in accordance with the Plan of Care.
- Uses a framework of assessment, implementation, coordination, and evaluation to provide outcome-based service for an assigned caseload from admission through discharge.
- Supervises Home Health Aide services assigned to his/her patients every 60 days as required by state licensing, ensuring compliance with the plan, effectiveness of the service, and patient/representative satisfaction.
- Supervises LVNs assigned to his/her patients at least every 60 days, as required by state licensing,, to evaluate service effectiveness and patient/representative satisfaction.
- Appropriately seeks supervisory assistance as necessary to effectively perform duties.
- Secures, documents, and coordinates physician orders for needed disciplines, medications, equipment, supplies, and community services related to the patient’s Plan of Care.
- Assesses and manages patient pain levels, providing symptom relief as necessary.
- Appropriately initiates preventive and rehabilitative nursing procedures.
- Maintains confidentiality of patients, personnel, and agency operations.
- Uses agency documentation and/or computer-based software to record patient service activity in a timely manner, including patient response and progress to goals. Documentation is completed at the end of assignments and submitted within 24 hours.
- Notifies the physician of any changes in the patient’s condition or needs.
- Complies with all home health care regulations, standards, policies, and procedures.
- Promotes patient, personnel, and environmental safety and utilizes universal precautions and infection management measures.
- Meets productivity requirements established for RN Case Manager: Completes a defined workload within a specified period, including managing a set number of patients, timely documentation (assessments, progress notes, plans of care), conducting the required number of patient visits and follow-ups, and ensuring that care meets clinical and regulatory standards while efficiently achieving patient care goals.
- Participates in record review as assigned.
- During patient/representative interactions, identifies cultural, spiritual, verbal, and non-verbal patterns that may indicate a need for individualized service or counseling.
- Assesses caregiver’s ability to manage patient needs and recommends respite or alternative options, if necessary.
- Evaluates the safety of the home environment and recommends solutions for identified concerns.
- Determines and educates the patient/representative on evacuation and disaster plans.
- Assesses equipment needs and safety, ensuring the proper functioning of equipment in the home. Reports any malfunctioning equipment to the appropriate agency immediately.
- Participates in on-call duties as assigned.
- Presents a professional appearance and promotes a positive work environment.
- Travels as needed to patients within Humboldt, Del Norte, Lake, Mendocino, and Sonoma counties.
- Performs other duties as requested.
QUALIFICATIONS:
- Graduate of an accredited school of nursing
- Possess one year of professional nursing experience; home health experience preferred
- Possess and maintain a current state license issued by the California Board of Registered Nursing
- Maintain CPR certification
- Possess effective communication skills and ability to work with individuals from a variety of backgrounds and educational experiences
- Possess good time management, nursing assessment, and work ethic skills
- Possess a current driver’s license and a dependable insured automobile
- Self-directed with the ability to make independent patient service judgments
- Demonstrate computer literacy
- Provide written proof of health status and ability to lift, bend, and squat at the level and amount determined by each job assigned
