PACE Medical Director Job Description
The PACE Medical Director is a progressive physician leadership role responsible for achieving the best clinical outcomes possible for all PACE participants. The Medical Director is responsible for the delivery of participant care, clinical outcomes, and the implementation as well as oversight of the quality assessment and performance improvement program.
This position requires excellent clinical, communication, and leadership skills spanning all areas of medicine, population health, social determinants, payer models, and compliance.
Key Responsibilities
- Deliver high-quality patient care and oversee the quality of care provided by other healthcare professionals.
- Develop and implement care pathways and clinical practice guidelines to effectively manage chronic health conditions and avoid preventable hospitalizations, emergency department usage, and premature skilled nursing facility placement.
- Collaborate with specialists including dentistry, podiatry, chiropractic, therapies (speech, physical, occupational), and medical subspecialties.
- Prescribe pharmaceuticals and treatment regimens as appropriate to assessed medical conditions.
- Provide on-call coverage for direct patient care after hours and weekends as indicated.
- Fulfill all administrative duties on a timely basis, including record keeping in participant charts, annual performance evaluations, corrective action plans, and strategic planning.
Requirements
- Doctor of Medicine (MD or DO) with an unrestricted license to practice medicine by the Oregon State Board of Medicine required.
- Required board certification recognized by American Board of Medical Specialties (ABMS).
- Certified Medical Director by the American Board of Post-Acute and Long-Term Care Medicine preferred.
- Minimum one-year experience working with the elderly or frail population required.
- Experience in a value-based healthcare organization, payer model, or closely related field preferred.
