Job Summary

The Re-Entry Coach plays an integral part of the Re-Entry program team by serving as the primary point of contact for individuals being released from incarceration and re-integrating into the community, working towards a smooth transition, and enhancing public health and safety. As a person with lived experience in incarcerated settings, the RC will help to identify health and social needs, create individual transition plans, and provide 1-on-1 support to ensure a smooth transition. Additionally, the PRC will identify new community resources and build on current local partnerships to connect clients to critical services.

Essential Functions

  • Serve as a peer, mentor, and primary contact for individuals throughout their involvement in the reentry program.
  • Assist with identifying health (physical, mental, behavioral) and social needs (including transportation, food, safety, neighborhood connectedness, housing, employment, etc.) and identify and eliminate barriers to addressing such needs
  • Address gaps of care by providing education, support, and connections to community-based resources
  • Assist with scheduling and coordinating medical and community service provider appointments
  • Provide comprehensive case management services as applicable (enrollment in Medicaid, SNAP, housing support, etc.)
  • Engage with individuals in conversation and education about substance use, harm reduction, treatment, and recovery to educate and reduce stigma around incarceration and substance use disorders.
  • Contact patients regularly to assess ongoing needs and progress towards established goals
  • Serve as a Packard liaison with partner agencies, providing referrals and warm hand-offs
  • Continually expand knowledge and understanding of community resources and uphold key community partnerships
  • Ability to be mobile during shift and travel between locations
  • Attend meetings and trainings as required.
  • Performs other duties as assigned.

Education and Experience

  • High School Diploma or GED.
  • Minimum of 1 year with no legal involvement
  • 1-2 years of experience working to facilitate resource navigation, preferred but not required.
  • CCAR, CPRC, or CHW certification, preferred but not required. Willingness to complete certification once hired.

  • A strong commitment to service, the mission of Packard Health and addressing the needs of vulnerable persons of the community
  • Ability to work autonomously in a fast-paced clinic environment in a courteous and friendly manner while maintaining the integrity of Packard standards.
  • Ability to use software as required while performing the essential functions of the job including but not limited to electronic medical records and Microsoft products.
  • Excellent communication skills in both written and verbal forms
  • Ability to work collaboratively in a team-oriented environment, courteous and friendly demeanor.
  • Ability to exercise sound judgment and problem-solving skills.
  • Knowledge of the local recovery community and resources is preferred.
  • Knowledge and understanding of multiple pathways to recovery.
  • Knowledge of harm reduction strategies and approaches to patient care preferred.

Job Type: Full-time

Pay: $16.53 - $24.63 per hour
