Job Summary
- The Social Worker provides direct clinical services within the Behavioral Health Department for inpatient adult civilian and forensic psychiatric units
- This position is responsible for conducting psychosocial assessments in accordance with DSM-V criteria, facilitating individual/group therapy sessions as assigned, identifying needed resources and referrals, coordinating discharge process (when applicable), and collaborating with interdisciplinary treatment team to ensure appropriate interventions
- Will report to Social Work Supervisor
Essential Functions
- Complete psychosocial assessment within 72 hours of admission
- Facilitate daily group therapy services as well as individual/family counseling services when applicable
- Participate in treatment team to evaluate patient’s progress in attaining goals. Consult with health care team members to promote, monitor, and evaluate compliance with patient’s treatment plan
- Provide appropriate psychoeducation to patient and family members on mental health issues and identify barriers to learning
- Coordinate resources both internal and external to address any barriers, ensure continuity of care, and maximize recovery plan. Acts as liaison between community resources, treatment team, family, and patient by conducting collateral contacts and referrals
- Coordinates discharge planning services and completes social services discharge summary within 24 hours of patient discharge
- Completes documentation in electronic medical records in compliance with quality standards
- Participates in social services team projects and staff meetings to enhance work environment
Required Knowledge/Skills/Abilities/Expectations
- Knowledge of evidence-based practice interventions for treatment of serious mental illnesses including crisis intervention, behavioral therapy, treatment milieu, suicide risks, co-occurring/substance use disorders and DSM-V diagnostic criteria
- Knowledge of individual, family, and group therapy treatment modalities
- Knowledge of community resources/agencies to assist with discharge planning
- Ability to conduct assessments, evaluate strengths and needs, and plan appropriate interventions
- Works effectively as member of multidisciplinary treatment team
- Ability to work under minimal supervision, take initiative, and use independent judgement
- Ability to manage rapidly changing priorities and deadlines based on current needs of units
- Demonstrated communication and problem-solving skills
- Ability to quickly develop and maintain therapeutic rapport with patients
- Sensitivity in working with diverse populations in acute psychiatric crisis including forensic patients
Qualifications
Education
- Bachelor of Social Work degree from a school of social work
Licenses/Certification
- Licensed by the Texas Board of Social Work Examiners as an LBSW, LMSW, or LCSW
- License must be current and without restrictions
- Basic Life Support (BLS) within 30 days of hire
- Handle with Care (HWC) within 30 days of hire
