The Community Office Assistant performs clerical, reception, and coordination of tasks to facilitate smooth operations of the geriatric primary care and palliative care businesses.
Community Office Assistant Position Duties & Responsibilities:
- Obtains necessary documentation for primary care and palliative care referrals and admissions, transfers, and discharges.
- Conducts chart audit daily and weekly to ensure proper demographic information is completed to allow for ease of reporting.
- Performs administrative tasks for electronic health record data entry including processing of referrals, admissions, transfers, and discharges.
- Coordinates and meets regularly with facility partners to ensure patient and facility needs are met, which includes faxing of all health records for primary care patients and scheduling of regulatory visits for geriatric primary care patients.
- Responds to requests for information in a timely, helpful, professional manner, making customer convenience and satisfaction a top priority.
- Assists medical staff with obtaining/releasing medical records, answering phones, calling patients as needed, running KASPER/INSPECT reports.
- Applies exceptional organizational skills in supporting Care Guide Partners staff and management.
- Creates a positive, compassionate, supportive, collaborative work environment for both co-workers and users of department’s services. Maintains collaborative and collegial relationships with IDT and other staff to assure coordination in delivery of care.
- Communicates clearly, concisely and in a timely manner with team members.
- Provides backup for reception services when needed at designated sites.
- Demonstrates knowledge of geriatric primary care and palliative care and can communicate the philosophy of care to patients and families, referral sources and the public.

- High school diploma required.
- Experience in an office-based setting.
- Demonstrates and maintains a high proficiency in computer applications and software programs. including Microsoft Outlook, Word, Excel, Power Point, electronic faxing, softphone/phone queues and accessing Sharepoint.
- Strong customer service skills and a commitment to providing excellent service to both internal and external customers.
- Demonstrated professionalism, integrity, and confidentiality in handling sensitive information and confidential documents.
- Strong organizational abilities, including the ability to prioritize tasks, manage time efficiently, and handle multiple projects simultaneously.
- Strong verbal and written communication skills.
