Palm Beach Accountable Care Organization is looking for a driven Commercial Project Management Consultant. A Project Management Consultant plays the role of a planner, advisor, and project manager to conceptualize and implement solutions that relate to quality or process improvements within the department.
Project Manager position is responsible for the following :
- Serve as the primary liaison between PBACO / Prominence and the participating provider(s) within our community who service our members in multiple product lines throughout Central and South Florida.
- Build and maintain strong relationships with providers by offering excellent support to assigned primary care and specialty care physicians with routine and targeted educational visits for the purpose of continued updates regarding policies and initiatives. Our service includes presenting actionable deliverables and prompt follow up.
- Conduct one on one on-site visits, virtual visits, telephonic conversations and written communications with individuals including Hospital executives, Patient Account Managers, Physicians, Office and Billing Managers.
- Perform on-site provider orientations, group orientations for new primary care, and specialty care physicians regarding policies, procedures, billing and company-wide initiatives.
- Represent the Physicians / Providers in workgroups and committees that address both internal and external concerns regarding operations as it pertains to the PBACO / Prominence network and the provider community.
- Applicant must be highly proficient and creative at identifying office workflow issues and problem solving. Organizations skills a must and Microsoft excel. Incumbent must be able to work independently as a telecommuter and attend off-site meetings.
- Focus on Quality as it relates to Commercial and Medicare Advantage Plans
- Abide by HIPAA requirements.
- Experience in program reporting and data analysis
- Flexibility - hours of work schedule based on department needs and deadlines.
- Other duties as assigned.
- Goal / Target Visits : Service primary care, and specialty care physicians within a specific geographic territory by conducting annual Goal / Target visits for the purpose of continued education regarding policies & initiatives. Each Provider Consultant will schedule minimally one goal visit per month with participating practice owners in the following specialties : Primary Care / Specialists.
- Directs multiple population health initiatives / projects simultaneously (e.g., clinical initiatives AWV, TCM, etc) within a healthcare network.
- Possess general knowledge of data analytics and reporting requirements.
- Responsible for creating monthly consultant projects and goals.
- Assist with preparation of presentations for new cost-savings initiatives.
- Responsible for developing monthly savings-driven projects and goals to accomplish.
- Improve department communication, presentation etiquette, and account organization.
- Present regularly to PBACO / Prominence team.
- Develop multiple trackers for PBACO / Prominence team.
- Ensure the scope and schedule of projects are achievable and undertake periodic review to make sure projects are on track.
- Update project documentation on a consistent basis and convey it to management.
