Role Description
This is a full-time hybrid role for a Practice Transformation Specialist located in West Palm Beach, FL, with some work-from-home flexibility. The Practice Transformation Specialist will oversee the implementation of healthcare delivery improvements, support practices in transforming healthcare delivery models, analyze practice data for quality improvement, and ensure compliance with ACO principles. The role involves collaborating with healthcare providers, conducting training sessions, and monitoring practice performance.
Qualifications
- Experience in healthcare delivery improvement and practice transformation
- Proficiency in data analysis and quality improvement initiatives
- Strong communication, collaboration, and training skills
- Knowledge of ACO principles and healthcare regulations
- Ability to manage projects and implement change effectively
- Experience as a Practice Manager is a Plus
- Previous experience in a similar role is advantageous
Salary : $55,000 - $65,000
