Palm Beach Nursing Center is Hiring a Full-Time Social Service Assistant!
Position : Social Service Assistant
Location : Palm Beach Nursing Center
Shift : Full-Time | 8 : 30 AM - 4 : 30 PM
Salary : $20 - $21 per hour
Responsibilities :
Participate in discharge planning, development, and implementation of social care plans and resident assessments.
Conduct interviews with residents and families to gather social history.
Coordinate with community resources to ensure residents have safe and effective transition plans upon discharge.
Continuously seek and integrate new methods and principles into existing social services practices.
Ensure that necessary forms, reports, and progress notes are properly completed with informative descriptions of services provided and residents' responses.
Maintain resident confidentiality at all times.
Know and comply with residents' rights and regulations.
Ensure the residents' emotional and social needs are met.
Job Qualifications :
Minimum of one (1) year of social work experience in a healthcare setting, directly interacting with individuals.
Preferred experience with Medicaid in a long-term care (LTC) or skilled nursing facility (SNF) setting.
Prefer a minimum of one (1) year related experience in a healthcare setting.
Completion of post-secondary education and the ability to read, write, and follow oral and written directions.
Must be capable of maintaining regular attendance as required.
Related social work experience at a level necessary to accomplish this position effectively.
