Key Responsibilities:
- Assess patient needs and develop personalized treatment plans.
- Collaborate with interdisciplinary teams to ensure seamless care transitions.
- Graduate degree in Nursing or a related field.
- Current licensure as a Registered Nurse and Nurse Practitioner in Maine (or eligible for reciprocity).
- Familiarity with Electronic Health Records systems (Epic preferred).
- Competitive salary range: $120,000 - $150,000 per year.
