Patient Liaison / Care Coordinator
The PALM Health Patient Liaison is responsible for administrative tasks, patient education, and care coordination, ensuring seamless medical and wellness services for PALM members. Serving as the primary point of contact for a designated physician, nurse practitioner, or multiple providers ("medical provider"), the Patient Liaison manages patient care by preparing the provider before appointments, overseeing the daily schedule, communicating changes or needs, and delivering patient education.
Following appointments, the Patient Liaison facilitates ongoing care by scheduling follow-ups, coordinating supplements, explaining lab collection instructions, and relaying messages between the provider and patient. Additionally, they provide guidance on lifestyle, diet, and medical recommendations. This role also serves as a backup for nurses (if qualified), medical assistants, and administrative staff as needed. Exceptional client service and hospitality are central to this position.
Required Qualifications
- Bachelors degree (B.S. or B.A.) or Licensed Practical Nurse (LPN) (required)
- Two years medical office experience (required); will consider other combinations of education and experience
- Experience with medical terminology and working in a clinic or hospital setting (required)
- Completion of coursework in medical terminology and medical records (preferred)
- Strong computer and word processing skills (required)
- EMR and scheduling experience (preferred)
