Area Coverage: San Jose, California
Starting per visit rate from $130.00.
As a PER DIEM Palmeira Home Health Physical Therapist, you can expect:
- Competitive, bi-weekly pay: Depending on Home Health experience
- FULL IRS Mileage Reimbursement: $0.67 per mile
- New Hire training for 1-3 days

Requirements:
- At least 1 year experience as a Physical Therapist
- Current licensure in the State of California
- Up-to-date Negative TB Test Result
- BLS CPR Certification
- Driver's License and Auto-Insurance
- Immunization/Vaccination Records (Covid19, Flu Shot, Hep B, MMR, TDaP, Varicella)
Others:
- Must be willing to drive and provide care in the patients’ places of residence
- Must be able to lift 25 lbs
- Must have no health restrictions that impede provision of care to the patient.
- With Home Health Care experience preferred
