Our team is growing, and Palmetto Family Homecare is looking for LPNs & RNs to join our team as Nurse Case Managers, managing the in-homecare of our clients within the TriCounty and Midlands areas, along with assistance in providing skills training for our employees.
This is position requires travel in your personal vehicle to visit clients in home, assisted living facility and/or in the hospital..
Key Responsibilities:
- Provide admission, case management, and follow-up visits for in-homecare patients.
- Conduct thorough assessments of patients' health status and create individualized care plans.
- Provide education and support to clients and their families regarding health management.
- Collaborate with interdisciplinary teams to ensure comprehensive care management.
- Conduct Quarterly Service visits with clients (Initial Visit, 30 day, 60 day, 90 day and Re-Evaluation)
- Maintain accurate documentation of patient progress and any changes in condition.
- Assisting aides with in-house skills training as needed
- Staff Cases with Care Services Team
- Attend Team Meetings and Education In-Services as required.
- Review of weekly service sheets, service plans and notes
- Patient 90 day and yearly assessment evaluations
- Assist with personal care services as needed to enhance client comfort.
- Foster strong relationships with clients and their families through compassionate communication.
Qualifications:
- Current nursing license in South Carolina or eligibility for licensure.
- Working knowledge of Microsoft Office Suite, Computer Skills and Resources
- Must be energetic, detail oriented, outstanding organizational skills and able to multi-task.
- Proven experience in acute care or home healthcare settings preferred.
- Strong knowledge of physiology and patient assessment techniques.
- Excellent patient service skills with a focus on compassion and empathy.
- Experience in case management is highly desirable.
- Familiarity with behavioral health practices is a plus but not required.
- Ability to work independently while managing multiple cases effectively.
- Must be a self-starter who enjoys working with the senior community, disabled adults & veterans.
