Job Summary
The hospice chaplain is responsible for offering spiritual counseling to hospice patients and their families for Hospice of Palo Alto County.
Job Status: PRN (As Needed)
Education/Requirements/Knowledge/Skills:
- The hospice chaplain will be available to provide spiritual counseling to hospice patients and their families, staying within their belief system.
- Maintains confidences of patients and their families.
- Documents in the clinical record progress notes in a timely manner.
- Uses effective communication skills.
- Attends IDT meetings and has input into developing, implementing, evaluating and documenting the plan of care.
- Reports necessary findings to hospice nurse if needed.
- Makes an effort to arrange visits of clergy and other members of the religious organizations in the community to patients who desire visits.
- Be available to individuals/families who do not have services from a spiritual person and request his/her services.
- Ordained and endorsed minister, priest, or member of a religious order or lay person with certified training in pastoral care.
- Pastoral services.
- Counseling individuals with terminal illnesses.
- Familiar/comfortable working with varied spiritual/religious practices
