Job Summary:
We are seeking a compassionate and dedicated Social Worker who desires to make difference in someone's lives to join our team, a not-for-profit organization. The Social Worker will be responsible for providing support, advocacy, and resources to residents and their families to ensure their overall well-being to meet the organization's mission, vision and values and in accordance with company policies and procedures and rules and regulations.
Pay Range: $30.15 to $38.81 per hour

Responsibilities:
- Conduct assessments to identify residents' psychosocial needs.
- Develop care plans in collaboration with residents, families, and healthcare providers.
- Provide counseling and support to residents and families to address emotional and social-related issues.
- Advocate for residents' rights and coordinate with external agencies as needed.
- Maintain accurate and up-to-date documentation of services provided.
- Participate in care conferences and team meetings to provide input on residents' care plans.
- Collaborate with other healthcare professionals to ensure holistic care for residents.
- Investigate grievances and alleged abuses.
- Assist residents with admission and discharge planning.
- Participates in Quality Assurance Program to achieve and maintain a 5 Star Rating facility.
Qualifications:
- Bachelor's or Master's degree in Social Work or healthcare related field.
- Previous experience working in a nursing home or long-term care setting is preferred.
- Strong communication and interpersonal skills.
- Ability to work collaboratively with a multidisciplinary team.
- Knowledge of social service resources and agencies.
- Proficient with database software, MS office products and office equipment
