What are the responsibilities and job description for the Hospice Nurse for PRN On Call position at Paloma Hospice and Palliative Care?
We are seeking an experienced Hospice nurse (RN or LVN) to join our team in the Round Rock/Austin, TX area. This PRN position is for an on-call rotation schedule and will require availability to work nights and weekends at least 2 weeks a month.

We are a growing company with offices in San Antonio, Austin, Del Rio, Dallas, Tyler and Sherman Texas. Our owners are clinical in origin and maintain that our main focus is on patients, their family, and the care they receive at this difficult time of life.
Responsibilities:
- Administer nursing care to ill, injured, or disabled patients
- Diagnose and establish patient treatment plans
- Monitor and report changes in patient symptoms or behavior
- Communicate with collaborating physicians or specialists regarding patient care
- Educate patients about health maintenance and disease prevention
- Facilitate referrals to other healthcare professionals and medical facilities
- Maintain accurate patient medical records
- Provide advice and emotional support to patients and their family members
Qualifications:
- Previous hospice experience in nursing or other medical fields
- Familiarity with medical software and equipment, Wellsky/Consolo experience preferred
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills
- Ability to thrive in a fast-paced environment

Pay:
PRN On-call rates include:
Hourly call rate: $3-4/hr
LVN's:
Routine Visit: $55.00
Death Visit: $75.00
RN's:
Admission: $150.00
Routine visit: $75.00
Death visits: $100.00
