Medical Case Manager

Qualifications: Bachelor’s Degree in Social Work, Counseling, Psychology, or related field. Prefer licensed or license-eligible social worker. The individual must be nonjudgmental, culturally sensitive, knowledgeable about HIV and skilled in case management and individual treatment service planning. Knowledge of local resources preferred. Must have considerable skills in locating, developing, and coordinating the provision of supportive services in the community, as well as skills in coordination and follow-up of medical treatments and support service referrals.
Experience: Experience in treatment case management or counseling, preferably in a public health setting or with high-risk populations.
Summary: The Medical Case Manager will provide guidance and assistance in accessing medical, social, community, legal, financial, and other needed services. Non-Medical Case Management services may include assisting eligible clients to obtain access to public and private programs for which they may be eligible. Will facilitate several methods of communication including face-to-face, phone contact, and any other forms of communication deemed appropriate on an individual case basis.
Essential Responsibilities:
· Conduct and facilitate case management process including intake, assessment, care plan development and implementation, coordination of services, monitoring/re-evaluation, and documentation.
· Complete and maintain documentation in accordance with Ryan White and agency case management standards.
· Develop written case management plan for each patient.
· Contact clients in a timely manner in the clinic, hospital, clients' homes and over the telephone.
· Educate and counsel clients about complex medication regimens, disease progression, and provides sensitive reinforcement of risk-reducing behaviors.
· Documents all HIV care coordination activities in individual client records.
· Assists with available health and human services appropriate for those who are HIV infected and helps providers with completing disability and medical assistance applications.
· Use motivational interviewing and harm reduction skills to assist in client success.
· Use excellent oral, written and communication skills.
· Establish rapport and maintain relationships.
· Increase and apply skill in harm reduction and trauma informed care.
· Maintain current knowledge of current standards of HIV/AIDS care and case management processes.
· Attend meetings, trainings, or conferences as requested or required.
· Other tasks or duties as requested or required.
