Summary
As a Physical Therapist Assistant at PAM Health at Home, you will play a vital role in delivering high-quality physical therapy services to our patients. This position is essential in helping individuals regain their independence and improve their quality of life through tailored rehabilitation programs.
Responsibilities
- Assist licensed physical therapists in implementing treatment plans for patients.
- Conduct therapeutic exercises and activities to enhance patient mobility and strength.
- Monitor patient progress and report any changes or concerns to the supervising therapist.
- Educate patients and their families about treatment plans and home exercises.
- Maintain accurate documentation of patient care and progress notes.
- Ensure compliance with safety protocols and best practices in patient handling.
- Collaborate with interdisciplinary teams to provide comprehensive patient care.
- Support discharge planning by preparing patients for continued rehabilitation at home.
Qualifications
- Completion of an accredited Physical Therapist Assistant program.
- Current state licensure or eligibility for licensure as a Physical Therapist Assistant.
- Strong knowledge of anatomy, medical terminology, and orthopedic principles.
- Experience in home health or outpatient settings is preferred but not required.
- Excellent communication skills to effectively interact with patients and families.
- Ability to work collaboratively within a healthcare team.
- Compassionate approach to patient care, particularly with geriatric populations.
- Familiarity with discharge planning processes is a plus.
