Job Duties:
I. Health & Safety
- Monitors health - general well-being and what is important for each individual, including possible side effects of medication; assists in the administration of medication in accordance with procedure.
- Ensures that residences are well maintained; reports any maintenance, health, or safety concerns promptly.
- Ensures that residential group homes are in full compliance with Home and Community-Based Services (HCBS) Settings Regulations;
- In a setting that is integrated into and supports full access to the greater community;
- Is selected by the individual from a variety of settings options;
- Ensures individual rights of privacy, dignity, respect, and freedom from coercion and restraint;
- Optimizes autonomy and independence in making life choices;
- Facilitates choice regarding services and who provides them.
- Requires that the person-centered planning process is directed by the individual or individual's chosen representative;
- Outlines the minimum requirements for person-centered service plans with individually identified goals and preferences.
II. Implementation of Supports Plan
- Ensures that interactions and supports for each individual demonstrate respect for the individual; allow for personal choice and recognize what is important to each individual.
- As assigned, provides support as indicated in the Individual Supports Plan including training to assist clients in meeting objectives, support and assistance as needed with personal care, domestic skills, community integration and/or related residential support activities; implements activity schedule.
- Provides behavioral support in accordance with Individual Supports Plan(s) and in compliance with both company policy and regulations.
- Provides transportation for appointments and activities; uses personal vehicle as may be required.
- Participates in interdisciplinary team meetings to discuss the development of the Individual Support Plan or the progress toward meeting stated objectives.
III. Documentation*
- Completes the required documentation of program activities and ensures that the individual's record is up-to-date and an accurate record of individual activity, behavior, and health.
- Maintains, as necessary, the required reports to support payroll and billing functions.
IV. Other duties as assigned; may vary by region.
Physical Requirements: *
- Position may require tasks such as lifting, running, and kneeling. Must be able to lift up to 50 pounds.
Special Requirements:
Required to complete federal background check and State of Virginia Central Registry check.
Valid driver’s license is preferred.
