1. Association Degree (COTA) from an accredited program
2. Current state Occupational Therapy certification
5. 1 years experience
Job Requirements
Current BCLS (AHA) certificate upon hire and maintain current.
National Board for Certification Occupational Therapy certification, AOTA and OTAC membership preferred.
Additional Details
American Heart Association or Red Cross is acceptable for BLS certification.
SUBMIT ANY NEW OR UPDATED COVID 19 AFTER ONBOARDING COMPLETED TO EMPLOYEE HEALTH UPON RECEIVING THEM.
Proof of 2 Vaccines or Positive Titer showing immunity required for MMR.
Annual Respirator fit test. APPOINTMENT REQUIRED WITH EMPLOYEE HEALTH TO COMPLETE ON SITE.
Declinations not accepted at Shasta Regional Medical Center for Hepatitis B.
Flu Vaccine or declination required October 1-May 31. MUST SEE EMPLOYEE HEALTH FOR DECLINATION.
MUST RECEIVE PROOF OF POSITIVE SKIN TEST AND PROOF OF CHEST XRAY applies for Positive PPD- Negative Chest X-Ray.
National Criminal Background Check within one year of start date. Exceptions apply.
