We are hiring Targeted Case Managers within the Central Florida area which include Orlando, Kissimmee and surrounding areas.
As a Targeted Case Manager, you will be an Independent Contractor and can work full-time or part-time creating your own schedule.
CBHCM (Certified Behavioral Health Case Manager) is a must.
Pay: $45,000.00 - $50,000.00 per year

Job Description:
Targeted Case Management Services consists of assisting recipients in gaining access to needed medical, social, educational, and other services. Targeted Case Managers shall link, coordinate, advocate, and monitor services per Medicaid Guidelines. The Targeted Case Manager is responsible for managing an assigned caseload of clients having mental illness and for assessing needs, developing, implementing, and reviewing service plans, and working with other community resources in meeting client’s service needs.
Duties:
- Provide case management services to individuals and families in need.
- Conduct comprehensive assessments to identify client needs and develop individualized service plans per Medicaid guidelines.
- Coordinate and collaborate with community resources to ensure recipients receive appropriate support and services.
- Monitor client progress and make necessary adjustments to service plans as needed.
- Advocate for clients and assist them in accessing necessary resources.
- Provide crisis intervention and support during emergencies or difficult situations. - Maintain accurate and up-to-date case records and documentation.
Requirements
- A bachelor's degree in human services, Psychology, Social Work, or related human services field is required.
- Completion of a training program approved by the Florida Certification Board.
- 1 year experience working with individuals who have complex needs.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong organizational skills with attention to detail.
