As a community outreach coordinator you will promote health within communities by conducting outreach to individuals that may be engaging in behaviors that place them at risk for contracting HIV. Outreach is to be conducted in the communities within Area 2A (Bay, Calhoun, Gulf, Holmes, Jackson and Washington counties). Identify, educate and provide Rapid HIV testing to individuals, and assisting them to adopt healthy behaviors. Serve as an advocate for the health needs of individuals by effectively communicating with them their risk factors, safer behavior practices and services that PanCare offers. Communicate with healthcare providers or social service agencies to meet the needs of these individuals. Act as a liaison or healthcare provider or social service agency to meet the needs of these individuals. Act as a liaison or advocate and implement the Ryan White early intervention service programs that promote, maintain and improve individual and overall community health. May collect data to help identify community health needs and conduct follow up as needed.
Responsibilities:
- Conducts outreach to identify and educate people living with HIV or at risk of contracting HIV.
- Provide HIV Rapid Testing for targeted, high-risk individuals.
- Ensure medical appointments are made for individuals testing reactive/positive for HIV.
- Provide support, counseling, recruitment and retention services for individuals testing positive including linkage to care for medical and other needed services
- Locates and follow up with individuals who have missed appointments or fallen out of care
- Maintain documentation for the number of individuals, activities and tests performed.
- Distribute educational materials and condoms during outreach activities.
- Prepares reports and statistical data.
- Assist Medical Case Manager in maintaining and updating client charts in ECW and CAREWare.
- Assist Medical Case Manager to make referrals for emergency and ongoing services within the agency and to other agencies as needed.

- Maintains liaison with community agencies and service organizations to determine the results of services rendered to individual clients.
- Some non-traditional work hours, (evening and weekends) required, including some day and weekend travel.

Required Skills/Abilities:
- Communication: Excellent verbal and written communication skills, including the ability to present information clearly and engage diverse audiences.
- Cultural Competency: Sensitivity and understanding of working with diverse populations, including those affected by HIV, and knowledge of health disparities.
- Program Management: Experience in planning, implementing, and managing community outreach programs; ability to collect data, track outcomes, and prepare reports.
- Interpersonal Skills: Strong relationship-building abilities to collaborate with community organizations, healthcare providers, and local leaders. Empathy and support for individuals with HIV.
- Knowledge of HIV Care: Familiarity with the Ryan White Program, HIV care services, healthcare navigation, and available resources.
- Outreach & Marketing: Experience with community outreach strategies, resource distribution, and raising awareness through various platforms.
- Organizational Skills: Ability to multitask, manage time effectively, and organize events and programs.
- Advocacy & Policy Awareness: Ability to advocate for policies that benefit individuals living with HIV and understanding of HIV-related legislation and healthcare rights.
- Problem-Solving: Critical thinking and resourcefulness to overcome barriers and address challenges in service delivery.
- Teamwork & Collaboration: Ability to work effectively with multidisciplinary teams and build strong community partnerships.
Education/Experience:
- A bachelor's degree in public health, Social Work, Communications, Healthcare Administration, or a related field is preferred.
- At least 2-3 years of experience in community outreach, public health, social services, or a related field, preferably working with underserved populations.
- Experience in HIV/AIDS care, case management, or working with individuals affected by HIV is highly preferred.
- Proven track record in organizing events, managing outreach programs, and working with community-based organizations.
- Experience in data collection, reporting, and tracking program outcomes.
- Familiarity with the Ryan White HIV/AIDS Program or similar public health programs is a plus.
Physical Demands:
- Sitting/Standing: Ability to sit or stand for extended periods during meetings, events, and outreach activities.
- Lifting/Carrying: Ability to lift and carry up to 25 pounds (e.g., event materials, promotional items, or equipment).
- Walking: Frequent walking to attend community events, outreach efforts, and meetings across various locations.
- Travel: Occasional local travel to community sites, healthcare providers, and outreach events. A valid driver’s license may be required.
- Visual & Auditory Requirements: Ability to read printed materials and engage with individuals and groups during presentations or events.
- Dexterity: Proficiency in using office equipment, computers, and technology for data entry, report preparation, and outreach activities.
