Position Details
Position: Full-time Admission Coordinator, Palliative Care
Schedule: Monday through Friday, 8:00 a.m. – 5:00 p.m.
Location: Indianapolis, IN, and surrounding areas

Requirements
Job Functions
- Supervise Palliative Intake Coordinator and Medical Records: Oversee the intake and medical records teams, promoting collaboration and ensuring alignment with Paradigm Health's values.
- Support Team of Nurse Practitioners: Provide resources and administrative support to nurse practitioners, enabling them to deliver the highest quality patient care.
- Coordinate Patient Follow-Up: Ensure seamless patient follow-up, maintain communication, and support continuity of care.
- Engage with Referral Sources: Work with intake team to gather required information for incoming referrals and maintain relationships with physicians and referral sources.
- Patient and Family Education: Offer compassionate education and support to patients and families on end-of-life care and palliative services.
- Maintain Professional Communication: Document referral statuses and updates in EMR, ensuring clear communication with all stakeholders.
- Proactively Support Office Needs: Triage calls, assist with patient scheduling, and contribute to a positive and efficient office environment.
Requirements
- Associates degree/ or Bachelor’s degree in healthcare administration, business, or a related field preferred (or equivalent experience).
- 3 years experience in a healthcare business development role or administrative role preferred, with 1 year in leadership.
- Strong organizational skills and attention to detail
- Advanced Proficiency in EMR and other software platforms (Excel, Word, Outlook)
- Excellent communication and interpersonal skills
- Valid driver’s license and reliable transportation
