Position Details:
- Part-time
- Schedule: 2-3 days per week (16-24 hours per week)
- Available territory openings: Lafayette Indiana and surrounding communities

Experience:
- One year of Nurse Practitioner experience is required
- Experience with geriatric population preferred
Job Functions:
- Provide medical support and coordination to palliative care patients and family members
- Complete history and physical examinations, by evaluating the patient's medical condition and health history
- Manage acute/chronic health conditions and terminal illnesses by developing an individualized plan of care, prescribing medications and treatments, obtaining consultations or making referrals
- Ability to adequately assess and reassess pain. Utilize appropriate pain management techniques. Educate patient and family regarding pain management
- Efficiently signs orders and scripts for medications for palliative patients
- Participates with provider on-call rotation
Credentials:
- Licensed N.P. in State of Indiana
- Proof of current classroom CPR
- Current driver's license
- Must have reliable transportation and insurance
