Job Description:
- Provide exceptional clinical care that includes skills such as wound care, medication administration, disease process education, IV therapy.
- Ensure clinical documentation demonstrates medical necessity and appropriately reflects quality patient care
- Submit OASIS and other clinical documentation in a timely manner
- Communicate patient's clinical needs with interdisciplinary team
Qualifications include the following:
- Current Texas RN License.
- Current Certification in CPR
- Valid Texas Drivers License and Insurance, with reliable transportation
- Experience with OASIS preferred
- Minimum of 1 yr prior Medicare home health experience, preferred
- Knowledge of WellSky (Kinnser) EMR is a plus
