We are actively seeking a dedicated Rapid Response Clinician to join our Rapid assessment and stabilization team at Paragon BHC. Job Description:
Rapid Behavioral Health Response Team - Clinician


Position Overview:

We are actively seeking a dedicated person to join Paragon as a Behavioral Health Clinician in partnership with the Jeffco Community Response Team. As a Clinician in this role, you will play a crucial role in assessing and admitting new clients based on clinical acuity and urgency. You will provide urgent behavioral health interventions to individuals in crisis and support clients who need support stepping down into or returning to the community. This position will also conduct outreach and engagement with identified members of the community and function on the team conducting assertive community treatment services and support to individuals needing ongoing support with activities of daily living, community stabilization, care management and vocational support. This role involves collaborating with multidisciplinary teams to develop safety plans, coordinating with emergency services, and connecting individuals with appropriate resources. You will also support crisis stabilization interventions and ensure smooth transitions to ongoing services provided by our in-home clinical teams or other teams as appropriate. This position requires excellent communication skills, the ability to remain calm under pressure, and a genuine commitment to helping those in need. Key Responsibilities:

  • Provide timely and empathetic responses to new clients needing expedited admissions into treatment, respond to urgent and crisis calls, messages, or situations.
  • Assess the severity of the crisis and collaborate with emergency/crisis services when necessary to ensure the safety of individuals in crisis.
  • Stabilize clients and then transition the individual to ongoing services such as IOP or intensive in home services.
  • For identified clients needing prolonged or ongoing support to stabilize situations, clients may be enrolled on an assertive community treatment team to maintain stability and sustain services for the clients as appropriate.
  • Connect individuals with appropriate resources and services, including mental health/substance use treatment programming and other professionals including social services, and community support.
  • Collaborate with the multidisciplinary team to develop integrated clinical formulations and safety plans and treatment plans.
  • Support crisis stabilization interventions, mental health holds, and evidence-based therapies tailored to individual needs.
  • Engage in Crisis Prevention and Intervention training and respond to behavioral health emergencies as needed.
  • Assists clients with the development of life skills such as maintaining personal hygiene, housekeeping, meal planning and preparation, meeting financial obligations, making and keeping appointments, and interpersonal skills.
  • Clinicians will conduct psychosocial assessments, treatment, service coordination, education discharge planning, and as required, urgent response to the most complex clients, frequently with co-occurring disorders, who may also have serious functional impairment in the domains of physical health and personal, family, financial, occupational, and social life.
  • Ensure accurate and timely documentation for clinical accountability, patient safety, regulatory compliance, and reimbursement.
  • Participate in reflective clinical supervision and engage in other professional development activities.
  • Provide support for clients' daily activities, fostering positive relationships and structured routines.
  • Develop service plans specifying goals and actions, ensuring participation, and identifying a course of action, including transition plan development.
  • Submit Progress Summaries to referring agencies with ongoing recommendations and maintain contact.
  • Maintain a comprehensive knowledge of available resources and stay informed about relevant community services.
  • Maintain confidentiality and adhere to ethical standards in handling sensitive information.
  • Foster positive relationships with external agencies, organizations, and community partners.
  • Facilitate M-1 holds as necessary.

Program Delivery Areas:

  • Urgent/Rapid Response: Ensure compliance with BHA standards, organized referral/response protocol, streamlined assessment and treatment planning, enhanced clinical training, mobile EHR access, and telehealth technology.
  • Assertive Community Treatment Team (ACT): Provide virtual access to nursing and psychiatry services, and meet ACT fidelity measurements.
  • Community Stabilization Programming (CSP): Interconnected with MCR, ensuring 24/7 coverage, extended hours during weekdays, and telephonic/virtual access after-hours.
  • Critical Incident Community Response: Developing collaborative partnerships for robust coverage and high-quality services for residents and visitors.

Behavioral Health Services:

  • Develop and implement a clear process to admit new clients based on clinical acuity and urgency.
  • Complete psychiatric diagnostic evaluations and initial treatment plans aligning with client goals.
  • Conduct individual, family, and group interventions and consultations with clinicians.
  • Ensure compliance with state regulations for comprehensive safety net organizations.
  • Leverage client strengths to prioritize interventions and immediate needs.
  • Assist clients in identifying appropriate clinical pathways and recommendations for the level of care.

Case Management and Supervisory Responsibilities:

  • Work as a multidisciplinary team with care managers.
  • Recommend and direct interventions and support appropriate for clients and families.
  • Encourage master’s level clinicians to work at the top of their licensure and leverage care managers for necessary community support services.
  • Identify and recommend solutions to multidisciplinary team members.
  • Direct care management staff in prioritizing urgent interventions at admission.
  • Provide ongoing consultation and supervision to staff working in communities and homes.

Qualifications:

  • Previous experience in crisis intervention, counseling, or a related field is highly desirable.
  • Strong communication and interpersonal skills.
  • Ability to remain calm and focused under pressure.
  • Empathy and a genuine desire to help others.
  • Knowledge of mental health resources and services.
  • Familiarity with crisis intervention techniques and protocols.
  • Ability to work flexible hours, including evenings and weekends.
  • Understanding of HIPAA, confidentiality, and protected health information.
  • Ability to work independently, prioritize assignments, and make rational decisions.
  • Strong conflict resolution skills.
  • Cultural awareness and commitment to diversity and equity.

Education:

  • Master’s degree in psychology, social work, counseling, or a related field.
  • Within one month of employment must obtain a preliminary license (i.e., Licensed Professional Clinical Counselor (LPCC), Licensed Social Worker (LSW), or Licensed Marital and Family Counselor Candidate (LMFTC) or similar.
  • All licenses must stay up to date and valid during employment.

Experience:

  • Two years of relevant experience in Behavioral Health and/or clinical service delivery is preferred.
  • A bi-lingual (Spanish speaking) applicant is preferred depending upon position location

Additional Requirements:

  • Valid driver’s license and proof of automobile insurance.

Hours:
Monday-Friday 10am-6pm


Salary: $60,000 - $75,000 for provisionally licensed staff (LPCC, CSW. LSW, LMFTC)

$75,000-$85,000 for fully licensed staff (LPC, LCSW, LMFT)
