Social Worker - Adult Day Care

```Duties```
- Provide social work services to residents in an assisted living facility or nursing home.
- Conduct assessments to determine the needs and preferences of residents.
- Develop and implement care plans to address the physical, emotional, and social needs of residents.
- Collaborate with interdisciplinary teams to coordinate and monitor resident care.
- Advocate for resident rights and ensure their well-being and safety.
- Provide counseling and support to residents and their families.
- Assist with meal preparation and medication administration as needed.
- Help residents with activities of daily living (ADLs) such as bathing, dressing, and grooming.
- Maintain accurate and up-to-date documentation of resident interactions and progress.

```Requirements```
- Bachelor's or Master's degree in Social Work or a related field.
- Valid license or certification as a social worker.
- Experience working in an assisted living facility, nursing home, or similar setting.
- Knowledge of resident rights and regulations governing long-term care facilities.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Compassionate and empathetic approach to working with residents and their families.
- Ability to handle heavy lifting and physically demanding tasks.

Job Type: Full-time

Pay: $30.00 - $55.00 per hour

Expected hours: 4 – 8 per week
