Home Care Per Diem Nurse

Paramount Healthcare Services is seeking Registered Nurses and Licensed Practical Nurses who are compassionate, kind and with excellent customer service, who are willing to go above and beyond their duties. Your duties would include participating in the planning and coordination of patient care in conjunction with the Clinical Director and the physician's plan of care and orders. (BILINGUAL and MULTILINGUAL STAFF ARE ENCOURAGED TO APPLY)

We are looking for you to be flexible to go either assignments directly in the town or in nearby towns.

Responsibilities

· RNs - Looking for staff to do start of care visits and complete appropriate paperwork and/or day-to-day visit with completed paperwork.

· LPNs - Looking for staff to perform day-to-day visit andcomplete appropriate paperwork.

· Participate in the planning and coordination of total patient care in conjunction with the DON and the physician's plan of care and orders.

· Follow physicians orders and ensure compliance with plan of care.

· Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change.

· Prepare clinical/progress notes and all required documentation.

· Provides and maintains a safe environment for the patient.

· Perform all duties and responsibilities, always adhering to company policies.

· Follow emergency procedures in the event of any catastrophic event. Example accident, injury or significant change in the patient's condition.

Qualifications

· Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing

· Current state license as an RN or LPN in good standing

· Current CPR certification

· At least ONE year experience as a Home Health Nurse

· Ability to pass Level 2 background & drug screening check

· Excellent verbal, written and interpersonal communication skills

· Excellent attention to detail

· Flexibility in work duties and schedule

· Ability to lift and move patients

Job Type: Full-time or Part-time

Pay:

RN - Oasis Start of Care: $95 per visit

RN - Recertification: $60 per visit

RN - Oasis Resumption of Care: $45 per visit

RN - Skilled Nursing visit: $45 per Visit

LPN - $40.00 per Visit

Job Types: Part-time, Temporary

Pay: $40.00 - $45.00 per hour
