The Memory Care Manager will oversee the daily operations of Memory Care, ensuring the highest quality of care and support for residents with Alzheimer’s, Dementia, and other memory-related conditions. Responsible for planning, developing, organizing, implementing and directing person-centered care programs and activities in Memory Care. Provide opportunities through a calendar of events using the company’s brand and program standards that enhance the physical, social, emotional, spiritual, intellectual and vocational needs of the residents. The Memory Care Manager must provide coordination, training and direction of the memory care / assisted living staff while fostering a compassionate, safe, and supportive environment for residents. Must be familiar with a variety of the field's concepts, practices, and procedures and ensure compliance with all regulatory and quality standards.

QUALIFICATIONS AND REQUIREMENTS :
- Must have a valid LVN license in the state of employment.
- 3-5 years of experience working in memory care or dementia care preferred.
- 2-4 years in a leadership role, with proven experience managing a team that provides person-centered care.
- Strong knowledge of dementia care principles, memory care best practices, and relevant state regulatory requirements.
- Excellent interpersonal and communication skills, with the ability to interact with residents, families and staff effectively.
- Ability to develop, implement, and assess individualized care plans.
- Experience with staff training, coaching, and development.
- Strong organizational and time management skills
- Ability to be part of the Monthly On-Call rotation
- Dementia care certification preferred (e.g., Certified Dementia Practitioner)
ESSENTIAL FUNCTIONS :
Tasks may include, but are not limited to the following :
Lead, mentor, and support a team of resident care associates in delivering person-centered memory care.Provide ongoing coaching, training, and performance evaluations to ensure high-quality care and adherence to policies.Be available for emergency or time-sensitive situations during your assigned on-call week; respond within the required time frame and take appropriate action in accordance with company policies and procedures.Meet with the Resident Care Director regularly to discuss plans, highlights, special challenges, required support, etc.Coordinate all programs and equipment use with Resident Care DirectorDevelop, implement, and monitor individualized care plans for residents, ensuring that all care is consistent with their needs and preferences.Assess residents’ physical, emotional, and cognitive condition and make necessary adjustments to care plans.Regularly interact with residents to ensure their needs are met and provide emotional support when needed.Responsible for successful operation of the department in coordination with the Resident Care DirectorAssesses Life History and Preferences for each resident; maintains Communication Book; takes reports from staff and refers clinical issues to the Resident Care Director for assessment and planningAt morning line-up, discuss residents who need to be monitored, staffing and programming issuesEnsure compliance with state regulations and internal policies related to memory care services.Maintain accurate and up-to-date resident records, including assessments, care plans, and progress notes.Monitor staff performance to ensure all documentation is completed thoroughly and timely.Act as a primary point of contact for family members and address any concerns or questions they may have regarding the care of their loved ones.Regularly update families on the well-being and progress of residents.Conduct team meetings to ensure open lines of communication and continuous improvement in care practices.Ensure that 12 hours of programming is provided, including evenings / weekends or per state guidelinesEnsure 1 outing per month is provided in Memory Care and coordinate special events with marketingCoordinate in-house special events open to families and friends (holiday parties, theme events, etc).Maintain the confidentiality of resident information and honor the residents’ personal property rightsEnsure a safe, clean, and welcoming environment for residents, visitors and staff.Observe and report changes in the resident’s functional status and report to the Resident Care Director and Care StaffBe familiar with community emergency procedures and be able to assist residents in case of emergencyMaintain clear and thorough communication with the General Manager and Resident Care Director and attend planning calls, meetings and training sessions as scheduledOther duties as assignedREQUIRED COMPETENCIES :
