What are the responsibilities and job description for the Hospice Chaplain/Bereavement Coordinator position at PARC Hospice?
PARC Hospice is looking to add a Spiritual Counselor/Bereavement Coordinator to our growing team. As a spiritual counselor you will Implement and coordinate the activities of the spiritual services to the patients and caregivers as needed. As the Bereavement coordinator you will develop and implement the Bereavement Program.
RATES
$40-$50 / per visit
Job Duties
- Spiritual/pastoral support is provided in accordance with on-going wishes and needs of the patient/family.
- Development of an individualized spiritual/pastoral plan of care which:
- Demonstrates an effort to work in close collaboration with local clergy whenever desired by the patient
- Provides spiritual support as needed and defined by the patient/family.
- Conducting religious services of prayer, worship and rituals for patients/families, as appropriate.
- Offering patients/families of different philosophies and religious beliefs opportunities to discuss and share their thoughts, feelings, beliefs and values.
- Develop educational programs and materials for patients, families, and staff and for the community on loss, grief and coping with bereavement
- Assures compliance of the bereavement department with all state and federal hospice regulations
- Facilitate annual memorial services.
- Attending IDT/IDG conferences when plan of care is reviewed.
- Travel to complete home visits and office meetings as indicated.
QUALIFICATIONS
- Graduate of accredited seminary or school of theology or appropriate certification of pastoral ministry
- Demonstrated experience in working with patients and families who have dealt with loss of a loved one.
- At least one year as a clinical social worker, marriage, family counselor or psychologist.
- Demonstrates clinical experience in the areas of grief and loss, individual family and group counseling
