At PARC Home Care and Hospice, we pride ourselves on providing compassionate, patient-centered care that consistently earns high praise from our clients and their families. In this Medical Social Worker (MSW) role, you will:

• Deliver Social Work Services: Provide social work support to patients in their place of residence, in accordance with physician orders and a defined plan of care.
• Act as a Resource & Consultant: Collaborate with an interdisciplinary team, guide staff on social service best practices, and connect patients/families with community resources.
• Contribute to Care Planning & Quality: Participate in developing plans of care, attend patient care conferences, document progress, and uphold quality improvement standards (QAPI).
• Support Patients & Families: Help address social and emotional factors related to illness or recovery, and provide coordination of discharge planning or additional community support.

Important Note: All team members are on-call every other weekend to ensure our patients receive continuous, high-quality care.

You Might Be a Good Match If You Are/Have

• A master’s or doctoral degree from an accredited school of social work (or a bachelor’s degree in a related field plus relevant social work experience).
• Experience in a health care setting (home care experience strongly preferred).
• Acceptance of PARC Home Care and Hospice’s philosophy and the ability to work collaboratively within an interdisciplinary team.
• Strong interpersonal skills: capable of exercising initiative, independent judgment, and building cooperation among patients, families, and colleagues.
• A desire to uphold confidentiality and safeguard patient information.

Pay: $48.66 - $53.95 per hour
