Wai'anae Coast Early Childhood Services (WCECS) is seeking compassionate Care Coordinators and/or Social Workers to provide Early Intervention (EI) services to families with infants and toddlers (0-35 months) with developmental delays at the Parent Child Development Centers in Wahiawā, Wai'anae, Waipahu, and/or Ewa Beach, locations. The CC/SW must be available to provide in-person services and attend virtual sessions/meetings. Compassionate, dedicated professionals are encouraged to apply.
Responsibilities/Duties
The CC/SW will be part of a multidisciplinary team providing services using the Primary Service Provider approach and coaching model to work with families in the home or community setting. This is a case management role with opportunities for utilizing advanced clinical skills in accordance with appropriate credentialling.
Job responsibilities include, but are not limited to:
- Provide information on the program
- Conduct assessments (intake, routine based, Battelle Developmental Inventory or Developmental Assessment of Young Children)
- Coordinate home visits and meetings
- Develop Individual Family Support Plans and goals for the child's development
- Timely and accurate documentation
- Participate in and facilitate team meetings
- Conduct in-person home visits
- Conduct tele-practice visits per client's discretion
- Connect families with needed resources
Required Qualifications
Minimum required qualifications: Bachelor's degree in Human Services, Psychology, Sociology, Social Work, or related field.
Other requirements of the position: Driver's license, and access to a car daily, and proof of vehicle insurance
Preferred Qualifications
Master's degree in Social Work and a Licensed Social Worker or Licensed Clinical Social Worker certification in the State of Hawaii.
Experience working with the pediatric population ages 0-35 months is highly preferred.
Physical Requirements
The staff member will be required to drive to and from the office, and client homes, and community locations per client discretion, as well as operate electronic and computing equipment, and operate videoconference platform ZOOM to attend required meetings, and to facilitate EIS sessions via tele-practice per client discretion.
Work Schedule
- Monday to Friday
- Generally 8am-430pm with rare later afternoons or weekends required in special circumstances.
- Manage your schedule allowing for required deadlines, team and staff meetings, and training.

Salary : $46,000 - $60,000
