Park Ave Nursing and Health Care Center is a short term rehabilitation and long term care center, seeking a social worker to join our team. The right candidate must possess a Massachusetts license as an LSW and have proven long term care experience. LTC experience is required .
Duties and responsibilities include and but are not limited to :
psychosocial assessments
completion of the MDS- mood, behavior and cognition sections
quarterly assessments
attend and provide input at care plan meetings
followup on complaints and grievances
provide ongoing support to residents and their families.
